  • One line: what this meeting was for
  • Decision needed? Yes/no — and who owns it

Decisions made

  • Each decision as a sentence — "X, because Y"
  • Who made the call, if it wasn't consensus
  • What was explicitly not decided

Action items

  • Task — owner — due date, one per line
  • Include the context that produced it ("send Sarah the revised scope" not "send scope")
  • Flag anything without an owner — it won't happen

Open questions

  • What was raised but not resolved
  • Who's answering it, and by when

Next time

  • Next meeting: date and what it needs to accomplish
  • Materials anyone promised to circulate first

Should notes be taken during or after the meeting?
